An Example Child Custody Excerpt...

The parents agree that it is in the best interest of the children that the Mother have sole physical custody of the children, born, unborn or legally adopted of this marriage. Both parents acknowledge that the actual primary physical residence of the children will be with the Mother and may be changed at anytime through proper court procedure or as they mutually agree. The parents agree that the Mother shall have control over the minor day-to-day decisions affecting the children.

The Mother agrees that the children should have as much contact with the Father as possible and that the children may visit the Father as often as may be agreed upon. Although possession time may be scheduled more often, the Father will have the right to be with the children on the following holidays and non-holidays:

An Example Relocation Excerpt...

The parents agree that in the event that one parent wishes to relocate, the other parent shall be given 90 days prior notice to assess the impact on this agreement. After that period of time, the parents agree to discuss the change in distance, change of schools, physical custody of child(ren) and ways of meeting the needs of both parents and the children.

An Example Child Support Excerpt...

If the Father, who is obligated to pay the child support, accrues a delinquency in amount equal to at least one month's support obligation, he will be subject to income withholding in an amount sufficient to satisfy the support obligation, and any additional amount to reduce and retire any delinquency.

The parents agree, that as long as child support payments are court ordered, the Father will carry and maintain life insurance in the amount of $100,000.00, naming the children as sole irrevocable beneficiaries. The Father will provide proof of such insurance coverage by means of providing a copy of the annual policy statement to the Mother.

The parents have attached to this agreement a completed child support calculation worksheet to further present to each other and the court the criteria used to determine the basis for the monthly support obligation amount.

An Example Dispute Resolution...

The parents agree to separate adult relationship issues from parenting issues, further, they agree to refrain from disparaging remarks about one another in front of the children or within the children's hearing range, including family members, friends and associates, that may estrange the children from the other parent.

Furthermore, the parents agree that the children will not participate in the process of resolving any disagreements or disputes regarding this agreement, unless mutually agreed upon by the parents.
